ASP practice question: Incident investigation
Question of the Day for Monday, 12 October 2026
ASP · Safety Programs and Concepts
A serious near-miss occurs with no injury. According to modern safety philosophy, how should it be treated?
Explanation
A serious near-miss should be investigated with the same rigor as an actual injury, because whether harm occurred was often a matter of luck - the same hazard could produce a serious injury next time. Ignoring it, recording without investigating, or waiting for repetition all forfeit a free warning. Near-misses are treated as seriously as injuries.
