Lockout / Tagout: Your Lock, Your Life
Never work on a machine unless your own personal lock is on the isolation point and you have verified the power is off.
Why this matters
- Unexpected startup of machinery or release of energy is a leading cause of fatal injuries on site. If someone flips a switch while you are inside or touching a machine, the result is often immediate and permanent.
- Relying on a verbal promise or a simple 'off' switch is not safety. In the Gulf heat, mistakes happen and communication fails. A physical lock that you control is the only way to guarantee the machine stays dead while you work.
Talking points
- Identify all energy sources: This includes electricity, compressed air, hydraulic pressure, and gravity.
- One Person, One Lock: Every worker must have their own unique lock. Never share your key with anyone, including your supervisor.
- The 'Try-Step' (Verification): Once you have applied your lock, try to turn the machine on. If it moves, the isolation has failed. You must verify it is dead before touching it.
- Use a Hasp for Multiple Workers: If more than one person is working, use a multi-lock hasp so the last person to finish is the only one who can enable the power.
- Release Stored Energy: Bleed off air pressure or drain fluids after the lock is on. A locked machine can still have 'hidden' energy in a spring or pipe.
- Tags are Mandatory: Your lock must have a Tagout with your name, photo, and phone number so others know who is working.
Ask the crew
- If five people are working on one conveyor belt, how many locks should be on the hasp?
- What is the first thing you should do after you put your lock on and before you start work?
- Who is the only person authorized to remove your personal lock?
